We are seeking a compassionate, organized, and detail-oriented Medical Assistant to join our Transitional Care Management (TCM) team. The TCM MA plays a key role in supporting patients after hospital discharge by coordinating care with our TCM RN's, scheduling follow-up appointments, and conducting weekly check-in calls. This role will also include cross-training for Medicare Annual Wellness Visits (AWVs) done over the phone.
